We focus on understanding individuals as a whole.
Through holistic case management, we help neurodiverse individuals build strong foundations for a life of independence after secondary school. This includes assessing all areas of life: from academics and physical needs to personal interests and therapeutic needs. Our process ensures that we can match individuals with the right programs and support they need for a fulfilling and independent adult life.
We’ve built our approach on these core elements:
Our specialized services help us guide individuals on any journey they’re on.
Transition Planning and Coordination
We assess individual needs, goals, and abilities in order to develop personalized and actionable transition plans that are right for you and your child.
Career Planning
Our approach combines personality insights with professional expertise to help you make informed decisions about your career journey. We conduct interest and skill assessments, such as the Myers-Briggs Type Indicator (MBTI) and Strong Interest Inventory, to match personalities to ideal career paths, enhance self-understanding, and create personalized strategies for individual success.
Educational Program Matching
We explore and match individuals with inclusive educational options after high school like traditional college options, Inclusive Postsecondary Education (IPSE) programs, and Career and Technical Education (CTE) programs. We also help families navigate the admissions process and ensure the right support services.
Ongoing Support and Advocacy
Throughout the transition process, we check in with families to discuss progress and answer any questions that arise to ensure confidence and ongoing success.
Independent Living Support
We help individuals achieve greater independence in their adult life by identifying suitable living programs, assisting with the transition, and assisting in the development of life skills.
